Below are some feedback on addition of information and some revision in the general DHS questionnaire:

Woman's Questionniare:
[REVISION]: In Q630: add NGOs in response column as NGOs also provide HIV services in significant scale
[ADDITION]: Suggest including 'distance for condom outlets' as new question [ADDITON] in this section.
Q929, 931 and 945: [REVISION] include NGO in the response section.
[ADDITION]: Suggest including 'distance for HIV testing sites'
[ADDITION]: Suggest adding questionnaire for 'Children with HIV and stigma': Current questionnaire that asks about female teacher doesn't capture the extend of stigma.
[ADDITION]: Suggest including questionnaire for 'Alcohol Use'


Man's Questionnaire
[REVISION]: In 721 this question is about female teacher. Why only female teacher? It would be appropriate to ask 'teacher' regardless of gender difference.
[REVISION} Q715 & 716 : suggest to include NGO in response section.
[ADDITION]: Suggest including 'distance for HIV testing sites'
[ADDITION]: Suggest adding questionnaire for 'Children with HIV and stigma': Current questionnaire that asks about female teacher doesn't capture the extent of stigma.
