You can get answers to questions like this at IPUMS DHS. At the website, I selected the Nigerian samples and searched for abortion. From the list of variables that turned up, I clicked on the name/link of the only one addressing whether a pregnancy ended in abortion. Then I clicked on the Survey Text tab to see precisely how the question was asked.
The way the questions were asked in Nigeria in 2008 and 2013 do not allow a researcher to distinguish purposeful abortions from other forms of pregnancy termination:
Have you ever had a pregnancy that miscarried, was aborted, or ended in a stillbirth?
