Many GIS studies seem to show that patients in LMICs are within 2 hours to a facility for surgery however, this is by using GIS modeling. We must have accurate patient data to know if patients really have access to surgery in a timely manner. Often the GIS assumptions do not take in the realities of how people travel to hospital in LMICs. https://gh.bmj.com/content/bmjgh/3/4/e000875.full.pdf.

Having grown up in the rural parts of an LMIC, I know all too well that a distance of 100 km to a hospital can mean a days travel. Gathering information from people faced with difficulties reaching a hospital within 2 hours would be a big step towards helping address their needs.

Patients lacking physical access to health facilities is a driving cause of mortality. This is an important indicator to help better understand the need for infrastructure development and emergency systems applicable to many conditions (trauma, obstetrics, cardiac care, sepsis, etc).

I am part of a research group in rural Rwanda. We are currently working on writing up our findings from a study to validate GIS mapping / modeling, by interviewing patients who underwent urgent / emergent C-section on travel time from home to district. GIS modeling consistently under estimated this time. GIS modeling from home to health center, and then health center to district hospital was a much more valid model. routinely understanding patient transit time will allow the health sector and infrastructure sector make improvements in this regard

Without data on distance for services, it is nearly impossible to properly allocate resources and attention to address one of the major hurdles to patients receiving timely access to care.
