I know that the NFHS-4 interviews took place between Jan 2015 to Dec 2016. I'm wondering whether households were tracked over the months of NFHS-4 period? In other words, did survey takers intentionally interviewed any particular individual/household more than once within a round of survey?

No households/individuals were interviewed more than once in NFHS-4. Because of the very large sample size, the fieldwork was conducted in two phases, but no households or individuals were revisited.
