The adjustment has changed over time. Originally v456 was adjusted for altitude only, but later it was adjusted for smoking too (as is the case in this survey). See the Guide to DHS Statistics page 138 for information on the altitude adjustments. Both adjustments are described in Adjusting Hemoglobin Values in Program Surveys.

Your second question is answered in the separate post. Also, just to avoid confusion the woman's weight is V437, not V478.
