Dear DHS Staff,
As I submitted my paper on one of the journals, it asked me the ethical review documentation. Can you provide that information for Nigeria Malaria indicator survey 2021?

(1) Give the name of the institutional review board or ethics committee that approved the study
(2) Include the approval number/date and/or a statement indicating approval of this research
(3) Indicate the form of consent obtained (written/oral) or the reason that consent was not obtained

Thank you so much, and I look forward to receiving those documents.
